The team works across Acute and Community hospital sites, General Practices, Mental Health services and a range of specialist teams. Partnership working is at the core of our values and we work closely with other members of the multi‑disciplinary team, including our Community Pharmacy colleagues, to deliver the highest quality care for everyone in Fife. As the Specialist Clinical Pharmacist, you will work closely with the Senior Clinical Pharmacist and renal multidisciplinary team to ensure safe, appropriate and clinically effective use of medicines. This includes communicating highly complex information relating to medicines with clinicians, nurses, non‑medical prescribers, carers and patients, as well as implementing and monitoring clinical pathways for treatments, including review of treatments in line with national guidance. You will assist in the management of patients with acute and/or deteriorating conditions as well as those in the dialysis units and outpatient settings, ideally working as an independent prescriber with a defined patient case‑load and attending ward rounds to maximise the pharmaceutical input into patient care. You will also have the opportunity to develop advanced clinical skills in renal medicine, including immunosuppressant management, therapeutic drug monitoring and dose optimisation for complex renal patients. This role offers involvement in service development, audit and research projects, contributing to innovation and best practice across NHS Scotland’s strong renal pharmacy network. You will contribute to the management of the pharmacy service to the medical specialties, ensuring optimum pharmaceutical care on admission, transfer or discharge and contribute to all aspects of clinical governance, education and training, medicine efficiencies, clinical risk and audit. We actively support professional development through NES programmes, independent prescribing training and advanced practice pathways. You will also play a key role in teaching and mentoring foundation pharmacists and trainees as well as sharing expertise within the wider multidisciplinary team.
