Care Coordinator II **BCForward is looking for position of Care Coordinator II (235515)** ***Position Title: Care Coordination Specialist** **Location: Remote - FL** **Office Location**: **8745 Henderson RD, Tampa, FL 33634** **Anticipated Start Date: ASAP** **Expected Duration: 03+ months of contract** **Schedule: Monday - Friday, 9:00 a.m. - 6:00 p.m. EST** **Potential to Extend: Yes** **Pay Rate: $25.00/ per hour** **Job Type: [FULL TIME (>%3D40 HRS WEEKLY, [CONTRACT], [REMOTE]** **Position Purpose**: Supports care management activities to ensure services are delivered effectively by healthcare providers and partners, achieving continuity of care and member satisfaction. Conducts outreach to members via phone or home visits, providing education and coordinating care with healthcare providers, community organizations, and caregivers. **Responsibilities**: - Conduct member outreach via phone or home visits to discuss care plans, next steps, and ongoing education. - Coordinate care activities with healthcare providers and community partners. - Serve as a support point for member and provider inquiries related to care plans. - Document and maintain member records in compliance with regulatory guidelines. - Assist with service assessments and screenings for members. - Identify and make referrals to Care Managers and other relevant entities as needed. - Provide education on available benefits and resources. - Ensure compliance with all policies and standards. - Must be comfortable with high-volume outbound calling (50-75 calls/day). - Flexibility to work from the office location if remote work is not feasible (e.g., due to power outage or internet issues). - Requires daily reporting of work completed (via Outlook). - Remote position, with quarterly in-person meetings. **Qualifications**: - **Required**: High School Diploma or GED; 1-2 years of relevant experience. - **Experience**: 2+ years in healthcare or managed care, Experience with high-volume calling, including outbound and inbound calls. - **Technology Skills**: General proficiency with Excel, Outlook, Word, and Teams. - **Preferred**: Bilingual (Spanish or Creole), previous work-from-home experience. **Benefits**: BCforward offers all eligible employees a comprehensive benefits package including, but not limited to major medical, HSA, dental, vision, employer-provided group life, voluntary life insurance, short-term disability, long-term disability, and 401k. **About BC**forward**: Founded in 1998 on the idea that industry leaders needed a professional service, and workforce management expert, to fuel the development and execution of core business and technology strategies, BCforward is a Black-owned firm providing unique solutions supporting value capture and digital product delivery needs for organizations around the world.
