Job SummaryThis is an exciting opportunity for a motivated and detail-oriented individual to join our team as a Retail Health Coordinator. In this role, you will work from our CVS HealthHUB location, focusing on the Wellness room. Under remote supervision, you will communicate with identified members telephonically regarding healthcare gaps, while adhering to emerging best practices and satisfying customer needs.
The Retail Health Coordinator will deliver excellent customer service and act as a navigator and coordinator to engage, assist, and promote initiatives relevant to healthcare gaps. You will coordinate patient care related to scheduling, organizing, and managing aspects of patient treatment and maintenance, arranging and managing patient care as directed by healthcare providers.
Key Responsibilities- Coordinate patient flow throughout scheduled appointments.
- Interact with customers/patients/members/coworkers telephonically and through technology-based interactions, including video, email, text, etc., and deliver healthcare reminders.
- Schedule, track, and reconcile patient referrals and diagnostic tests as necessary.
- Demonstrate proficiency in managing email in an Outlook account and using remote communication software such as Skype, Microsoft Teams, and WebEx.
- Adhere to care management, privacy, and confidentiality processes in compliance with regulatory guidelines, company policies, and procedures.
- Provide superior customer service to internal and external clients, customers, and patients.
- Communicate with clinical teams, patients, and families regarding recommended patient care plans.
- Notify clinical care teams of any changes or barriers encountered as necessary.
To be successful in this role, you should possess:
- A high school diploma or equivalent.
- Minimum 3-5 years of experience in healthcare, customer service, patient relations, and/or hospitality concierge roles.
- Strong computer, data entry, and typing skills, along with the ability to work between multiple technology platforms.
- Proficiency in Microsoft Office applications, including Outlook, Word, and Excel.
- Excellent customer service and communication (verbal, written, listening) skills.
The estimated annual salary range for this position is $43,000 - $56,000, depending on experience and qualifications.
