Estimated Salary Range: $60,000 - $80,000 per year, depending on experience and location.

Job Description: We are seeking a dedicated and experienced Social Worker to join our team. The ideal candidate will have a strong background in customer service and be passionate about making a difference in the lives of others.

Responsibilities:

• Assess patient needs and develop personalized plans to address psychosocial, grief, financial, environmental, and community needs
• Provide individual patient/family counseling, crisis intervention, and information assistance
• Facilitate access to community resources and support services

Requirements:

• Bachelor's or Master's degree in Social Work from a state-accredited program
• Current state license required; one year of experience as a Social Worker in a clinical care setting preferred
• Demonstrated excellent communication and interpersonal skills
